They tighten everything up, giving you a longer time of performance but also heightens the sensitivity of it too. Great fun.I have never tried one what do they actually do and which is best if i am going to try ?

Agreed, don’t waste decent money (£20-30 up) for a decent thick steel one if it’s not for you.I'd suggest you buy either a silicone or an adjustable leather one first and try that out.
