Yes. Was nasty actually. long story short, more sexually flirting than anything and brought us together.
I was using a very large carving (professional) knife in work when a certain someone slapped my arse for a laugh.
I missed what I was cutting and went through the top of my finger. It was hanging off.
Ended up having several stitches, nerve blockers the lot. All the while, HRH happily watched the surgeons repair my finger.
So while not strictly sexual accident on par with your post, it was sexual flirting that caused it.

I can't think of anything off the top of my head other than the odd scratch from a finger nail or head bump from use judging a head board.
